MASSAGE CANCELLATION REQUEST

We require 3 hours prior notice of cancellation for massages rendered in our office. If we do not receive cancellation 3 hours prior to your scheduled massage you will be charged $20 for a 30-minute massage that you scheduled and $40 for a 60-minute massage that you scheduled and failed to notify us 3 hours prior to your massage time.

Thank you for understanding, as our massage therapists do not get paid unless services are rendered.

By checking this box, I understand the massage cancellation policy and will incur the fee should I notify the office less than 3 hours prior to scheduled massage appointment.
